BUCKLE trial: on the SGEM (episode 415)

This month I am back on the SGEM podcast – this time on the all new Paeds section with Dr Dennis Ren.
In this episode we do a deep dive into the BUCKLED trial that looked at the use of bedside ultrasound vs. plain film X-rays for the initial investigation of paediatric forearm fractures.
This is a really neat Australian trial run by my friend Dr Peter Snelling and friends in Queensland.
Bottom line: ultrasound appears to be non-inferior to X-ray when used as a first line investigation in terms of functional, patient-oriented outcomes.

And so fast! I didn’t use to have the confidence to drop X-rays but now only do them if cortex is disrupted or if near the growth plate. Nice to not have to get the X-ray tech out of bed. Parents love being able to make the diagnosis themselves too.
